In Maryland, individual and group insurance plans that cover pregnancy-related benefits must include infertility treatment, offering up to three IVF cycles per live birth, with a lifetime maximum of $100,000. Coverage applies to married and unmarried individuals who meet specific criteria, including a history of failed attempts at conception or certain medical conditions. IVF must be performed at clinics following ASRM and ACOG guidelines. Fertility preservation is covered for medically induced infertility, but storage of sperm or oocytes is excluded. Employers with fewer than 50 employees, self-insured employers, and religious organizations are exempt.
